New Patient Registration

Bring in your medical card. If you cannot find it, speak to one of our reception staff who will give you a registration form (GMS1) and a new patient medical questionnaire to complete.

We will check it and, if we agree to accept you as a patient (e.g. if you live within the area we cover) we shall then arrange for you to have a new patient health check which involves a 20 minute appointment. We would like you to bring a small urine sample in a clean, sealed bottle.

Your registration form will be sent to the Health Authority and you will receive confirmation that you are registered with us.

Medical treatment is available from the date of registration. Please contact reception for further information.
